Prior to signing your legal contracts and escrow being funded you may have expenses for items related to traveling for medical screening, prenatal vitamins, medications, copays or insurance. Receipts are required for reimbursement and can be uploaded at the bottom of this form.

Please use this form to notify your Case Manager of a necessary reimbursement so that we can be sure it is received in a timely manner for you. Reimbursements are made within two business days and a Paypal or Venmo account is required for reimbursement.
